As a convenience to our patients we offer the opportunity for you to review our policies and complete new patient paperwork at home. Please bring the paperwork with you to your scheduled appointment. At your first appointment, our staff will answer any questions you may have regarding this information and review the completed paperwork with you. If there is any additional information you require prior to your appointment, please call our office and we will be happy to assist you. If you need to reschedule or cancel your appointment, please call us at least twenty-four hours in advance. This will allow us the courtesy of offering your appointment to another patient. Complete the patient information paperwork. Bring the paperwork with you 20 minutes prior to your scheduled appointment time. Please DO NOT mail back the paperwork. Completing this information ahead of time allows us to see you in a timely manner upon your arrival at the office, and ensures we have the information necessary to fully address your health care needs.
In addition, please bring the following items with you:
Your completed paperwork.
A driver’s license or other photo identification. *
Your insurance card(s) and/or claim information.
A referral (if required by your insurance plan).
Insurance co-pay.
Copies of pertinent imaging studies (x-ray, MRI, CT Scan, Arthrogram, Myelogram or Bone Scan), if taken
outside the Hanover Hospital.
Copies of any previous orthopaedic medical records (include Report of Operation).
The name and phone number of the physician who referred you to be seen by HOA.
*Photo Identification: If photo identification is not available to document the current address, please present a utility bill or other correspondence showing current residence. If the patient is a minor, a parent or legal guardian is asked to provide such information.
Our office hours are Monday thru Thursday 8:30AM – 5:00PM. If you have an emergency after hours, please call our main number at 717-632-5259 and our answering service will contact the on-call physician for you.
What to Expect During the Visit:
Upon arrival, you will be greeted by our staff and your registration information confirmed. If your plan requires a co-payment, it will be requested during check out. Hanover Orthopaedic Associates accepts cash, personal check, or debit, Visa, MasterCard and Discover as forms of payment for your convenience. Self pay patients will be asked for a $200 deposit at registration and any balance of the visit at check out.
At your initial visit you may be asked to change into a disposable gown or shorts to prepare for the medical evaluation. If you require x-rays our office is equipped to perform radiographs ordered by our providers. Afterwards, you will be escorted to our check out and, if needed, schedule your next appointment. We will be happy to aid you in scheduling any additional tests or physical therapy. If elective surgery is warranted, you will speak to a member of our staff to proceed with the scheduling process.
What to Expect After the Visit:
Our office will submit a claim to your insurance carrier(s) on your behalf. If there are remaining balances after the insurance carrier(s) have paid, we will send you a statement. Please refer to the financial policy tab for more information.
