The following plans require a referral and/or authorization prior to your appointment. If a referral is not presented to our office at the time of service, you will be responsible for payment in full.

Please note that even though we are providers for the above listed insurance plans, your particular plan may consider our charges or diagnosis as non-allowable. If any services, durable medical equipment or diagnoses are considered being non-allowable, the guarantor is responsible for those charges.
Insurance and Payment
Please bring your most current insurance card(s) to your appointment. If your insurance plan requires a referral or authorization, you must obtain this document/information from your primary care physician before your appointment. We will file insurance claims for you. If our services are not covered by your insurance, payment in full is expected at the time of service. Self pay payments will be asked for a $200 deposit at registration and the balance for services at check out. For your convenience, we accept MasterCard, Visa, Discover, cash and personal checks.
